Terms of Service

1. Introduction

These Terms of Service (“Terms”) govern the use of services provided by My Health & Wellness Medical Group, located in Miami, FL. By accessing our website, booking an appointment, or receiving medical, wellness, or aesthetic services, you agree to comply with these Terms in full.

2. Eligibility

● Services are available to individuals aged 18 and above.

● Minors under 18 may only receive services with the consent and presence of a parent or legal guardian.

3. Appointments, Cancellations & No-Shows

● Appointments can be booked in advance via our scheduling platform (Acuity) or by calling our office.

● To cancel or reschedule, please notify us at least 24 hours in advance.

● Cancellations made less than 24 hours before the appointment, or failure to appear, may result in a cancellation fee.

● We reserve the right to refuse or reschedule appointments if medically necessary or if the patient is not an appropriate candidate for the requested service.

4. Payments

● Payment is due at the time of service unless otherwise arranged.

● Accepted forms of payment include Zelle, credit/debit cards, cash, HSA/FSA (where applicable), and approved financing partners.

● All prices are subject to change without notice.

5. Health & Safety

● Patients must complete a medical history and intake form prior to receiving care.

● For medical and wellness services, you may be required to undergo evaluations or provide lab results.

● MHW reserves the right to decline treatment if it is deemed unsafe or medically inappropriate.

6. Telehealth & Digital Services

● Some services may be offered via telemedicine in accordance with Florida law.

● By using our telehealth platform, you consent to electronic communication and acknowledge that telemedicine has limitations compared to in-person care.

7. Gift Cards & Vouchers

Valid for 12 months from date of purchase.

● Non-refundable and non-transferable.

8. Personal Belongings

MHW Medical Group is not responsible for the loss or damage of personal items brought into our clinic or treatment areas.

9. Conduct & Respectful Behavior

● Patients and guests are expected to maintain respectful behavior toward staff and other patients.

● MHW reserves the right to refuse service to anyone engaging in abusive, inappropriate, or disruptive conduct.

10. Privacy & Confidentiality

● All patient information is handled in accordance with HIPAA regulations.

11. Limitation of Liability

To the fullest extent permitted by law, MHW Longevity Center is not liable for any indirect, incidental, or consequential damages arising from the use of our services, including but not limited to injuries, delays, or inability to access services.

MHW may update or amend these Terms at any time. Updated Terms will be posted on our website, and continued use of our services constitutes acceptance of the revised Terms.

13. Governing Law

These Terms shall be governed by and construed in accordance with the laws of the State of Florida.
